FIELD TRIP PERMISSION SLIP

I hereby grant permission for my son/daughter/ward _____________________ to participate in a summer field trip to Greece from July 5 2084 - July 12 2084. I understand this field trip is optional and attendance by my child is not required, nor will it affect my child's position or grade at Hogwarts School of Witchcraft and Wizardry if my child elects not to participate.

Transportation for this field trip will be provided by portkey.

The planned field trip is tentatively scheduled to leave the South End of Diagon Alley at 10:00 AM, JULY 5 2084 and return to the South End of Diagon Alley at approximately 5:00 PM, JULY 12 2084. A letter will be sent to parents/guardians if a decision has been made to arrive or dismiss students to parents/guardians at an alternative location.

I understand that all students participating in this trip will be responsible in conduct to the teachers and adult sponsors at all time. I further understand that students are required to go and return from this event on the transportation provided, unless prior arrangements have been made.

Authorization to treat a minor: In the event that I cannot be reached in an emergency, I hereby give permission to the physician selected by the school staff to secure proper treatment for my child.

I understand that the Hogwarts' Board of Governors provide up to 50 percent financial compensation for this trip with the provision that all persons making a field trip or excursion of their own choosing shall be deemed to have waived all claims against the Board or the School for injury, accident, illness or death occurring during or by reason of the field trip or excursion.

OOC: It's field trip time of year again! We'll be gathering at the South End of the Street in Diagon Alley starting August 10, tentatively moving to the field trip thread by August 13. Sign-Ups will be open until a thread in the Vanishing Cabinet is created to continue the field trip RP, so make sure you post here before then, or risk missing out! If your character will be a 5th-7th year during the Term 38 school year, feel free to sign up! Your character must attend Hogwarts for the 2084-2085 school year to participate.
