OOC: The following form is meant for those here for an illness or injury. All visitors may proceed through without checking in.

Upon entering the Hospital Wing, you come upon a tall, wooden podium with a sign attached to the front of it. "Check-in Area for Patients," the sign reads, alerting you to the fact that Healer Ashford wishes for you to document your medical condition in the book spread out across the top of the podium. It's already opened to the next available page, so go ahead and grab the provided quill and sign in before you proceed to the Waiting Room for Patients and Visitors. This way, Healer Ashford is sure to not miss anyone, and all those ill or injured can be properly seen and tended to.

OOC: You may check-in in character or out of character (if your character's condition is too severe), so please utilize the thread even if your character is in serious condition.

Please fill out the following form and post it in this thread. You'll only need to post in this thread once for each medical visit.

NOTE: After filling out the form and posting it in here, please post your character in the Waiting Room for Patients and Visitors. I usually try and PM you when it's your turn to see the Healer, however, in case I accidentally forget, just continue to watch the Waiting Room, please.

Character's Condition and/or Symptoms: Over the summer, Azrael fell out of a tree, due to an owl that had come to deliver his Hogwarts letter. His parents visited a muggle hospital, seeing as they are muggles, to treat his broken arm and 4 broken ribs.

Anna: Waiting on PM.

Allison walked in now limping. She had been attacked by Robin, and it was partially her fault. The baby white owl was temperamental and she didn't blame him. However with all the running she did after Robin, and then walking back up the stairs towards the Owlery, and then back down towards the Hospital wing by herself, Allison was exhausted, hurting and bleeding more than necessary with such small pecks. The owl hadn't really attacked her too hard, simply that his beak was strong and her skin too sensible. It hadn't even hurt at first. Grabbing the quill Allison signed her name and what was going on with her ankles, making sure to leave out the part of Robin being offended.

The reason im posting for her is because her charrie does not know about her condition and my character is supposed to take her to the healer.

Character's Condition and/or Symptoms:Rafe just got back from Romania, where he underwent "training" and has taken quite a beating. He's managed to heal a few of his scars on his face, but his chest and back has a few large gashes as well as badly bruised. It looks as though he's been attacked by some sort of animal :shifty: He's not in a big hurry to see the Healer and can endure the pain, he just is seeking for something quick and easy to help with the comfort a bit. Plus, he's heard a rumor that the new Healer is quite a looker! :lol:

Character's Condition/Symptoms-Rex was hit by a hayword branch of the Whomping Willow while standing to close. Not noticing the branch, it knocked him out. He now cannot remember anything except for a few select names that he forgets soon after saying them. So in other words, amnesia.

Lucille saw the note in front of her and let out a slightly defeated sigh. There was writing involved? That would mean writing left handed, and that - well, that was going to be a disaster. She approached the board, clutched the quill uncertiantly in her left hand and began to scribble, tounge stuch between her two front teeth.

Lucille squinted at the form. It was just about ledgible if you half closed your eyes and tilted your head to the left....but it would have to do. She turned and progressed to the waiting area.

Still feeling very much betrayed, Jake sidled up to the sign in thing and stared at it hesitantly. He grabbed the quill and then wrote out what was needed in his attrocious left-handed scrawl, the words spelt phonetically with backwards letters. It took a while before he was done and could move on.
