While we do not fully understand all facets of the biomagical processes involved in magically converting fear into sustenance, we do know that some dark creatures are able to break down emotional energy the same way a wizard can break down food into energy units, metabolizing it and using it to fuel their biological reactions. That being said, many wizards and muggles alike are unable to break down certain food components, such as lactose, causing a bad physical reaction. The boggart also has such an intolerace.
